Fellowship Application

Applications are invited with Filled Application Form, the CV and Brief Description in 250 words of work done for consideration along with nomination by 2 UPDA life members.

Last date for receiving application form is 31st July

  • Three applications will be selected in academic category and Two in social category.
  • Each application has to be nominated by 2 UPDA life Members.
  • Each member can nominate only 2 applications.

Criteria for consideration

  • Involved in Diabetes care for minimum 5 years.
  • Should be UPDA member for minimum 5 years.
  • Special contribution to Diabetes care and UPDA.
  • Minimum qualification: MBBS/MD/DNB.

Award Categories

A. Academic

This award shall be given to a life member of UPDA based on his academic achievements related to treatment, research, patient education or prevention in the field of diabetes.

  • The person should be more than 45 years of age.
  • Must be proposed and seconded by life members.
  • Bio data in 4 copies along with Photostat copies of three original papers.

B. Social Service

This award shall be given to a life member of UPDA based on social service related to treatment, patient education, or organization of diabetes camps and conferences.

  • The person should be more than 40 years of age.
  • Must be proposed and seconded by life members.
  • Bio data in 4 copies along with proof of social services.
  • Provide contact details of two local referees and two outside city referees.

The Credential Committee will have the right to verify his achievements form these referees and independent sources while taking into consideration for the award of fellowship of UPDA.

Fellowship shall be awarded during special convocation held during annual conference of UPDA. The citation of fellowship shall be read out by the Secretary/Joint Secretary, or any person nominated by the President of UPDA and shall be awarded by the President. Both fellowships will consist of a similar certificate and a medal.

Application Fee: Rs 10,000/- (To be submitted after approval from Credentials Committee)
